    Explosions are uncommon but can occur when internal faults produce rapid gas generation and pressure rise, especially with arcing, severe overheating, or bushing failures. If protective devices fail or operate too slowly, tank pressure can exceed mechanical limits. Contributing factors include degraded insulation, high moisture, poor connections, OLTC issues, and combustible gas accumulation. Pressure relief devices, fast protection, and oil condition monitoring reduce the likelihood and severity.
